I would like to ask of you what you have done to investigate my repeated reports of emotional abuse to [daughter], daughter of Tracy C, and a ward of the Crown under the responsibility of your agency.
In summary of the report, I have repeatedly notified you, Barbara Mackinnon, your staff supervisors, Ministry officials Kathy Neff, Judy Dunnington (Program Supervisors), MPP Richard Patten, the Premier, Daulton McGuinty, and several media reporters, that Ned Jackson, a supervisor in your agency, is threatening to intentionally cause severe harm to [daughter]'s emotional well-being by threatening to terminate visits with her natural mother T.C, whom [daughter] loves very much, has repeatedly stated so, as well as reports from her own access visit supervisors who have repeatedly noted are beneficial to [daughter] also.
As a professional in a position of authority over children, it is your duty to report when a child is in need of protection, whether is be from the actions of your own staff, or otherwise, and to ensure that [daughter] is protected from further emotional, physical, or sexual abuse and neglect.
By not acting on this report, you are putting yourself in a position of contributing to her emotional abuse, which makes you legally liable.
