Is it morally ethical for a Canadian who has been living in the United States for a few years and thus not paying Canadian taxes to return to Canada and use the health system?

Legally, the rules vary from province to province. In Ontario, if they have given up their residency in Canada, they have to wait 3 months upon return before they become eligible for OHIP again.

These can be complicated situations.
There are lots of people resident elsewhere who continue to pay different kinds of Canadian taxes, for different reasons.
Certainly people who still own property here do, although that's another way of saying that class may be giving some people an advantage. I know of a case like that and it gets my back up a little, especially because the people involved are right-wingers and usually mock Canadian social-democratic tendencies -- right up to the point where they want to come back and use our healthcare system.
